How should I ask a counselor for a recommendation for the University of Hawaii application?

I’m a high school junior/senior and I’m starting to put together my college applications. I want to ask my counselor for a recommendation, but I’m not totally sure what I should say or how to make the request feel respectful and clear.

I’m trying to keep it simple and make it easy for them to write a strong letter.
